EQUIP is an improvement and learning team supporting sustainable change across North East London since 2014.
Combining quality improvement methods, Population Health Management (PHM), facilitation and system insight, EQUIP works with practices, neighbourhood teams and partners to translate priorities into practical improvement.
Built on strong local relationships and over a decade of experience, EQUIP supports both practice-level and system-wide change.
